Need for Baseline Assessment
A baseline assessment and organized assessment tool are essential to effectively address social and economic conditions. By establishing a baseline, we can understand the existing SDOH factors that impact communities, enabling targeted interventions and resource allocation for maximum impact.
It is crucial to address these social and economic conditions to improve individual and population health outcomes. By providing resources and interventions, we can positively impact the well-being of individuals and communities, reducing health disparities and fostering health equity.
Why Social Determinants of Health Matter
SOCIAL DETERMINANTS OF HEALTH (SDOH) are the conditions in the environments where people are born, live, learn, work, play, worship, and age that affect a wide range of health, functioning, and quality-of-life outcomes and risks -- (Healthy People 2030).
Greater Influence
SDOH have been shown to have a greater influence on health than either genetic factors or access to healthcare services. They play a pivotal role in shaping individuals well-being and overall health outcomes. (CDC 2022)
Impact of Inequalities
Poverty and other inequities have a significant impact on health outcomes and contribute to an increased risk of premature death. These disparities in access to social and economic benefits, such as housing, education, wealth, and employment, perpetuate health inequities. (CDC 2022)

Our Objective
Create an international SOCIAL DETERMINANTS OF HEALTH client baseline assessment tool to assist in providing services to developing and rural communities.
Who We Are?
We are the International SDOH Baseline Assessment Project, a collaborative initiative aimed at creating tools to enhance the quality of life for rural and developing communities.
What We Do?
We work collaboratively to create tools to support enhancing the quality of life for rural and developing communities.
Where?
And we are based in Addis Ababa, Ethiopia, our reach extends globally.
Our Approach
Many existing tools for assessing SDOH lack standardization, scoring mechanisms, and recommendations for interpreting individual-level results. Additionally, they often fall short in representing aggregated data, hindering comprehensive analysis and decision-making.
Intake Form Design
We have developed an intake form specifically designed for the baseline assessment of SDOH. This form serves as a crucial step towards developing a peer-reviewed international survey tool.
We used validated SDOH screening tools such as; PRAPARE, DHS, Structural Vulnerability Assessment Tool, Health Begins Up Streams Risk Screening Tool, WellRx Toolkit, Kasier Permanentes Your Current Life Situation Survey and American Academy of Family Physician (AAFP) Social Needs Screening Tool, through the preparation of the intake form below.
On this Intake form we have tried to address the different contexts in both urban and rural settings. We have also tried to address the gaps of existing tools. Our intake form will give an evidence to make decisions on feasible interventions.
